Insurance Information

We accept all dental PPO plans regardless of network status.

Network participation varies by provider and location and may change periodically. Please contact your preferred location to get current network information.

We currently only accept dental HMO in our Plainfield office and only with Dr. Roh. We accept Cigna, Sostice, and United Healthcare HMO policies. Any other HMO company is NOT currently accepted. 

Please keep in mind that for HMO you need to select a dental home with your insurance company. Make sure you contact your insurance company to make sure we are listed as your provider before scheduling an appointment. Failure to do so may result in uncovered visits.

We DO NOT accept Medicare. However, we do accept dental supplement plans that are PPO. 

We are a part of the Tricare Dental Network. We can see anyone who is enrolled with Tricare, including active duty and reserve sponsors and their covered family members.

If you are an active duty sponsor and your visit requires an appointment control number, please provide that number to us on the day of your appointment in order for your claim to be covered. Also please let us know if you will need a DD2813 form and we will be happy to complete one for you. 

We currently DO NOT accept Medicaid, Public Aid, All Kids, or any other state provided dental insurance. Our offices simply are not large enough to support the necessary requirements to handle the patient load that comes with these types of insurances. We are not contracted, therefore we cannot make any exceptions. 

If you are having a hard time finding someone who will accept your insurance, the best thing you can do is reach out to your insurance company directly for assistance. 

The only dental discount plan that we accept are Cigna Dental Savings Plans in our Plainfield office with Dr.Roh only. Any discount plans with other companies will not be accepted.
